CBC Summer League Forms
These two documents need to be completed for each athlete participating in any Cedarburg Summer League Games. These digitized documents replace the need for the paper copy waivers previously signed in-person at the CHS gym during week 1 of play.
Summer 2026
This registration is for parents or athletes (18 years old or older) to complete the necessary Waiver and Code of Conduct prior to participating in Cedarburg Summer League Games.
There is no cost to this registration, only two forms to be completed.
